Our day services in Shrewsbury

All of our day services have been designed to offer a number of different services on different days to provide choice and flexibility to you.

If you have been referred to our day services by your GP, District Nurse, hospice Outreach team or other healthcare professional involved in your care, a member of our team will get in touch with you to discuss the next step.

This is me Living Well – eight-week assessment

Patients who have been referred to our day services for assessment will have a named nurse who will guide you through an eight week programme. During this time we will explain the full list of services available and discuss with you which services may be of benefit.

Currently the eight-week assessments run on a Tuesday and Wednesday each week between 10am to 3pm.

Living Well clinics

Our specialist palliative care clinics, managed by our doctors, physiotherapists, nurses and complementary therapists run on a Monday. Each clinic focuses on a specific area, for example lymphoedema, MND and complementary therapy, and run on a weekly, fortnightly or monthly basis. If you have been referred to one of our clinics we will get in touch to arrange a convenient appointment for you.

My Well-being

We have a number of different specialist services designed to support your well-being including a range of educational sessions covering a number of themes including managing fatigue and sleep, mindfulness, managing fear, health and living well, plus many more. These sessions take place on alternate Thursdays between 10.30am to 1pm. As these sessions are patient steered if there is a specific topic you would like to cover we will try to arrange this for you. If you have been referred to this element of our day services we will get in touch to discuss which sessions may be of benefit for you.

Living Well Together

Living well together are informal drop in sessions run on alternate Thursdays between 10.30am and 1pm. These sessions are designed to be more of a social gathering that anyone with a palliative diagnosis can attend who might need some advice or would just like to have a cup of coffee in a relaxed setting. Although we do not provide hands on nursing care during these sessions members of our multi-disciplinary team will be on hand to answer questions, they will also be able to refer you to another service if your circumstances have changed. Alongside our multi-disciplinary team other local community and voluntary services will attend the sessions to provide help and information.

Our day services are provided as part of your overall care, and will compliment your continued care at home.
