Hospice at Home

Our Hospice at Home service supports patients who are in the final stages of their illness to die at home, free from pain and surrounded by those closest to them.

The team provides hands-on nursing care to help patients spend their final weeks with dignity and in peace. We will also support carers and family members through their time of greatest need.

What to expect

Once you have been referred to our Hospice at Home service, a member of the team will get in touch with you to discuss your needs. We will arrange for one of our nurses or healthcare assistants to visit you at home.

We will visit at agreed times to spend time with you. This could be during the day or night and for different lengths of time depending on your needs. All of these decisions will be agreed with you when you are referred so you know what to expect.

We are happy to answer any questions you may have to help reduce the stress that can be felt during what is usually a difficult and worrying time.
