WHO ARE WE?

We are a team of qualified and experienced Social Workers, and are all registered with the General Social Care Council (www.gscc.org.uk). Between us we have a wide range of skills and knowledge.Our bereavement service is also supported by a team of Bereavement Support Volunteers who we train and supervise.

 

WHAT CAN WE OFFER YOU?

The Team provides a high quality Social Work service to:

    •   In-patients
    •   Day-unit patients
    •   Patients at home
    •   Relatives, friends and carers
  We work closely with the multidisciplinary Hospice team and can liase on your behalf with the community based professionals such as Macmillan nurses and Home Care team. local authority social workers, charities and welfare organisations, home care agencies, housing associations, department of work and pensions.

    • Counselling
    • Facing and making decisions
    • Communication within the family
    • Coping with loss and change
    • Helping children understand loss
    • Planning for the future
    • Information and advice -for example about care at home, how to find the right nursing home, sources of possible financial assistance
    • Liasing on your behalf

Or ask any nurse, doctor or other member of the Hospice Team to contact us for you. We will be in touch with you as soon as possible.

CONFIDENTIALITY

The hospice operates on a team based approach to your care.  We may sometimes need to share information you give us with our colleagues to ensure you are receiving the best service.  We will always endeavour to check with you about what we may or may not divulge.
