Hospice Outreach Team AIMS OF THE SERVICE To provide specialist knowledge, advice and holistic care to those suffering from a terminal illness whether at home or in hospital. Also, by acting as a resource and making this specialist knowledge available to all Health Care Professionals and statutory and voluntary agencies involved in providing palliative care for those patients and their carers. OBJECTIVES OF SERVICE
ACCESS TO THE SERVICE This is via the patient’s GP, Hospital Consultant (with GP consent) or District Nurse (with GP consent), completing the appropriate referral form. HOURS OF WORK Monday - Friday 9am - 5pm . However, these hours will be flexible in order that patients and other users of the service may receive an optimum service. Outside of these hours, advice will be available by contacting the Hospice. REFERRALS Criteria for Acceptance: 1. All patients with a diagnosis of cancer. 2. Initially other terminal progressive illnesses will be accepted on to the Outreach Team’s ‘books’ at the discretion of the Hospice Medical Director and Nursing Director. 3. Referrals are made by completing the Hospice referral
form and forwarding this to the Hospice. In emergency situations
telephone referrals will be accepted. NB: Patients already having regular contact with Macmillan Nurses will not be visited by a nursing member of the Hospice Outreach Team unless at the specific request of the Macmillan Nurse involved. CONTACT DETAILS Depending on which GP Surgery the patient is registered with, on referral to the Hospice the patient will be assigned a Hospice Outreach nurse. Their names are: -
